  • Nursery - Kinder P400
    Grade 1 - 3 P450
    Grade 4 - 6 P500

  • The diagnostic test will help us assess your child's current academic level and identify any areas that may need additional support.

Requirements to Prepare

  • Nursery 1 (3 years old by Oct 31)
    Nursery 2 (4 years old by Oct 31)
    Kinder (5 years old by Oct 31)

    • Filled out Enrollment Form

    • Diagnostic Test

    • PSA Birth Certificate

    • 2x2 recent photo of the child

    • Report Card (if available)


  • Grade 1

    • Filled out Enrollment Form

    • Diagnostic Test

    • PSA Birth Certificate

    • 2x2 recent photo of the child

    • SF 9 with LRN (Form 138 Report Card)


    Grade 2 - Grade 6

    • Filled out Enrollment Form

    • SF 10 (Form 137 Learner's Permanent Academic Record)
