    SF 9 with LRN (Form 138 Report Card)


    Grade 2 - Grade 6

    SF 10 (Form 137 Learner's Permanent Academic Record)

  • Online Submission

    For online submission of requirements, simply upload your documents and email them to discoveryintegratedacademy@gmail.com*

    *name the subject with your child’s full name and level. (e.g., Chloe Garcia, Kinder)

  • In-person submission

    Please bring your documents to Discovery Integrated Academy located at 104 B&C, Shaw Blvd., Pasig.

    We are open from Mondays to Fridays from 8 am to 5 pm. Walk-in submissions are welcome, and our friendly staff is happy to assist you.
